Nikki started her medical career in 2006 as a surgical tech while getting her BS in Exercise Science. In 2014, she received her Master’s degree in Physician Assistant Studies from the University of Utah, which is the 2nd most prestigious PA program in the nation.

Sarah White is a dedicated and experienced Physician Assistant with a diverse background in functional medicine, family practice, emergency medicine, and neurology. She earned her undergraduate degree in microbiology from Brigham Young University (BYU) and completed her Physician Assistant degree at Trevecca Nazarene University in Nashville.

Alison Blake, FNP, graduated with her Master’s Degree in Nursing from Westminster University. She is a board-certified Family Nurse Practitioner through the American Association of Nurse Practitioners. She has over 20 years of Medical Experience.

Dr Amanda DuBois is a Family Nurse Practitioner who is passionate about working with patients to find root causes of disease instead of symptom-based treatment. She focuses on an individualized and patient-centered approach that empowers patients to gain control of their health. The overall goal is to address the underlying causes of disease and promote optimal wellness.

Kecia was born and raised in Portland, Oregon. She attended Oregon Health Sciences University, where she received her Bachelors of Science in Nursing. She practiced many years of her nursing career in Women’s Health and Labor and Delivery.

Melanie attended Weber State University where she received a Bachelor of Science in Nursing in 2008. While working in the Mother/Baby unit, she completed her Master of Science in Nursing at the University of Phoenix.

Heather received her Masters Of Science In Physician Assistant Studies from the University Of Utah in 2014 and has been in the healthcare industry for over 20 years.

Cale received his MBA from the University of Phoenix and has 20 years experience in business operations. Cale is also a Hormone patient himself, after turning 40 he was experiencing common symptoms of low testosterone.

Carrie is a Utah native, mom of three, and grandma to two cute little girls.  She has a passion for health and wellness.

Certified through NASM and currently working on additional certifications specializing in Women’s weight loss, Shauntel is thrilled to be a part of the team. She lost 100 pounds 20 years ago and loves helping others find health.

Stephanie has her masters in Aesthetics and holds both a phlebotomy license and a CNA license. She has over 10 years of experience in the medical field providing top notch patient care. 

Madi helps ensure all things at Revitalize go smoothly. From gathering lab results, to organizing DEXA events, she does a little bit of it all. She takes the lead on managing our social media, as well as our blog and newsletters.

Camille works closely with our providers and pharmacies to ensure that patients to ensure the prescriptions refill process goes smoothly.

Tammie is originally from Georgia. Prior to coming to Revitalize Tammie worked on the Brain Tumor Team at Huntsman Cancer Institute as a Patient Coordinator. Tammie loves talking with patients and is passionate about creating an amazing experience for them.

Aleisha is our patient liaison with new and potential patients and has extensive experience in customer service.

Andrea is our receptionist at suite 102B. She handles the majority of the phone calls and welcomes our patients when they come in the office.

Originally from the Pacific Northwest. Alayna moved to Utah after graduating with a Bachelors in Marriage and Family Studies.

Zoie is our receptionist at suite 101B for our lab and DXA's. She has 7 years of customer service and has a passion for healthcare and wellness.
