Please read these Terms of Service (“Terms”) carefully before using revitalizemedicalsolutions.com (the “Site”), operated by Revitalize Medical Solutions (“Revitalize,” “we,” “us,” “our”). By accessing or using the Site, requesting a consultation, or completing our Hormone Quiz, you agree to these Terms. If you do not agree, please do not use the Site.

1. No Medical Advice; No Doctor-Patient Relationship

The Site, including our blog, research summaries, FAQs, and Hormone Quiz, is provided for general informational and educational purposes only. It is not medical advice and is not a substitute for a diagnosis, treatment, or recommendation from a qualified healthcare provider. Hormone Quiz results are informational only and are not a diagnosis. Nothing on the Site, and no communication through our forms, chat, or email, creates a doctor-patient (or provider-patient) relationship. That relationship begins only once we have confirmed an appointment and you have completed our intake process. Never disregard professional medical advice or delay seeking it because of something you read on the Site. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.

2. Eligibility

You must be at least 18 years old to use this Site, submit our forms, or request services on your own behalf. If you are submitting a request, completing our Hormone Quiz, or otherwise providing information on behalf of a minor patient, you represent that you are the minor’s parent or legal guardian, that you have the legal authority to consent to health care services on the minor’s behalf, and that you consent to being contacted regarding the minor’s care. Separate consent to treatment, signed by a parent or legal guardian, is required before we provide services to any patient under 18.

3. Free Consultations and Scheduling

Submitting a consultation request or completing the Hormone Quiz does not guarantee an appointment or a specific outcome. We will contact you to confirm scheduling. Consultations are informational and do not constitute treatment. Fees, cancellation policies, and financial terms for actual services will be disclosed separately once you become a patient.

4. Hormone Quiz Disclaimer

Our Hormone Quiz is a self-assessment tool based on your answers to general symptom questions. It is not a validated diagnostic instrument, does not review your full medical history or labs, and should not be used as the sole basis for starting, stopping, or changing any medication or treatment. Always discuss quiz results with a qualified provider.

5. Testimonials and Results

Patient testimonials and stories reflect individual experiences and opinions. Individual results vary, and no testimonial is a guarantee that you will experience the same or similar results.

6. Communications and Marketing Consent

By submitting our Contact Us form, Free Consultation request, or Hormone Quiz, you consent to be contacted by phone, text message, and/or email regarding your inquiry, as described in our Privacy Policy. You may opt out of marketing text messages and emails at any time; instructions are provided in our Privacy Policy and in each communication.

7. Acceptable Use

You agree not to:

use the Site for any unlawful purpose;
submit false or misleading information through our forms or Hormone Quiz;
attempt to gain unauthorized access to the Site, our Patient Portal, or related systems;
scrape, harvest, or reproduce Site content without permission; or
interfere with the Site’s operation or security.

8. Intellectual Property

All content on the Site – including text, graphics, logos, the Revitalize name and marks, and the Hormone Quiz – is owned by or licensed to Revitalize and is protected by copyright and trademark law. You may view and print Site content for personal, non-commercial use only; no other use is permitted without our prior written consent.

9. Accessibility

We are committed to providing a Site that is accessible to all users, including individuals with disabilities. 10. Patient Portal

Our Patient Portal is hosted by a third-party vendor and is subject to that vendor’s own terms and security practices in addition to our HIPAA Notice of Privacy Practices. These Terms govern the public marketing Site only, not your use of the Patient Portal once you are an established patient.

11. Affiliate Links and Third-Party Content

Our Products page includes affiliate links, including Amazon Associate links, from which we may earn a commission on qualifying purchases at no extra cost to you. We do not control, and are not responsible for, third-party websites, products, or services linked from the Site, including social media platforms.

12. Disclaimers

THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE, OR THAT ANY HEALTH OUTCOME, WEIGHT LOSS, OR OTHER RESULT DESCRIBED ON THE SITE WILL APPLY TO YOU.

13. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, REVITALIZE MEDICAL SOLUTIONS AND ITS OWNERS, PROVIDERS, AND STAFF WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE SITE. THIS SECTION DOES NOT LIMIT ANY LIABILITY THAT CANNOT BE LIMITED UNDER APPLICABLE LAW, INCLUDING LIABILITY FOR ACTUAL MEDICAL CARE PROVIDED TO PATIENTS, WHICH IS GOVERNED SEPARATELY BY YOUR PATIENT CONSENT AND TREATMENT AGREEMENTS.

14. Indemnification

You agree to indemnify and hold harmless Revitalize Medical Solutions and its owners, employees, and agents from claims, losses, or damages arising out of your misuse of the Site or violation of these Terms.

15. Governing Law and Venue

These Terms are governed by the laws of the State of Utah, without regard to conflict-of-law principles. Any dispute arising from these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Salt Lake County, Utah.

16. Changes to These Terms

We may update these Terms from time to time. Continued use of the Site after changes are posted constitutes acceptance of the revised Terms. The “Effective Date” above reflects the most recent revision.

17. General Provisions

If any provision of these Terms is found unenforceable, the remaining provisions will remain in full force and effect. These Terms, together with our Privacy Policy, constitute the entire agreement between you and Revitalize regarding your use of the Site and supersede any prior agreements on that subject. Our failure to enforce any provision of these Terms is not a waiver of our right to do so later. We may assign these Terms at our discretion; you may not assign these Terms without our prior written consent.
