This Privacy Policy describes how Revitalize Medical Solutions (“Revitalize,” “we,” “us,” or “our”) collects, uses, and discloses information through revitalizemedicalsolutions.com (the “Site”). It applies to visitors to the Site and to prospective patients before they become established patients. It does not apply to our Patient Portal (operated by a third-party practice-management vendor) or to protected health information created once you become a patient – that information is governed by our HIPAA Notice of Privacy Practices, available on our website and in our office, not by this Privacy Policy. The Site and our services are intended for residents of the United States; if you access the Site from outside the United States, you do so on your own initiative and are responsible for compliance with applicable local laws.
1. Information We Collect
Information You Provide to Us:
- Contact and consultation requests: name, email address, phone number, and any message or details you provide when you submit our Contact Us form or request a Free Consultation.
- Hormone Quiz responses: if you complete our Men’s or Women’s Hormone Quiz, we collect your answers to symptom-related questions (e.g., sleep, energy, mood, weight, libido) along with any contact information you provide with your results.
- Communications: information you provide when you call, email, or message us.
- Testimonials/reviews: if you submit a testimonial for us to publish, we collect what you submit and, unless you ask us to anonymize it, your name as provided.
Information Collected Automatically:
- Usage data: pages visited, links clicked, time on page, referring/exit pages, browser type, device type, and approximate location derived from IP address.
- Cookies and similar technologies: we use cookies and tags, including Google Tag Manager and Google Analytics, to operate the Site and understand how it is used. See “Cookies & Tracking Technologies” below.
2. Important Note on Health Information
Basic contact details submitted through our general Contact Us form (name, email, phone) are not, by themselves, protected health information. However, if you submit information about your symptoms, medical history, or reason for seeking care – including through our Hormone Quiz or a consultation-request form – that information may be treated as protected health information under HIPAA once we receive it, even if you have not yet had your first appointment. We handle that information in accordance with our HIPAA Notice of Privacy Practices. This information is transmitted to and stored in our customer relationship management (CRM) platform. We require appropriate agreements and safeguards with our CRM vendor consistent with HIPAA. Any automated email or text message campaigns are configured to avoid referencing specific symptoms or health details.
3. How We Use Information
- To respond to inquiries and schedule free consultations and appointments.
- To provide, maintain, and improve the Site and our services.
- To send appointment reminders, follow-ups, and requested information.
- For marketing and advertising, provided this does not involve using Hormone Quiz or intake health responses for advertising purposes without your authorization.
- To analyze Site traffic and usage trends.
- To comply with legal obligations and protect our legal rights.
4. Cookies & Tracking Technologies
We use Google Tag Manager to deploy Google Analytics on the Site for general traffic and usage analytics, such as pageviews, session duration, and referring sources. This analytics tracking is aggregate/traffic-level only. The content you submit through our Contact Us form, Free Consultation request, or Hormone Quiz – including any symptom-related answers – is transmitted directly to our CRM platform, and is not shared with Google Analytics or any advertising pixel. We do not currently run advertising or remarketing pixels (such as Google Ads or Meta/Facebook Pixel) on the Site. If that changes in the future, we will update this Policy and put appropriate safeguards in place before doing so. You can still control general analytics cookies through your browser settings or by using Google’s Analytics opt-out browser add-on. Our Site does not currently respond to browser Do Not Track signals.
5. How We Share Information
We do not sell your personal information. We may share information with:
- Our customer relationship management (CRM) platform, which receives and stores the information you submit through our Contact Us form, Free Consultation request, and Hormone Quiz, and which we use to follow up with you and manage our patient pipeline.
- Our patient portal and practice-management vendor, if you create a portal account (subject to that vendor’s own privacy practices).
- Other service providers who help us operate the Site or provide analytics services, under confidentiality obligations.
- Legal and safety purposes: to comply with law, respond to legal process, or protect the rights, property, or safety of Revitalize, our patients, or others.
- In connection with a merger, acquisition, or sale of business assets, as permitted by law.
6. Text Message and Email Communications
If you provide your phone number or email address through our Contact Us form, Free Consultation request, or Hormone Quiz, we and/or our CRM platform may contact you by phone, text message, and/or email regarding your inquiry, appointment reminders, and, if you do not opt out, informational or marketing content about our services. Message frequency varies; message and data rates may apply to text messages. You may opt out of text messages at any time by replying “STOP,” and you may unsubscribe from marketing emails using the link in any email. Opting out of marketing communications will not affect appointment-related or account-servicing messages necessary to provide services you have requested. Consent to receive text messages or marketing email is not a condition of receiving services from Revitalize.
7. Affiliate Disclosure
Our Products page contains affiliate links, including Amazon Associates links. As an Amazon Associate, we earn from qualifying purchases. We may earn a commission if you purchase products through these links, at no additional cost to you. We only recommend products we believe may be useful to our patients and readers; inclusion of a product is not a guarantee of its effectiveness or safety for your specific situation.
8. Your Privacy Choices and Rights
Depending on where you live, you may have rights under state privacy laws (such as the Utah Consumer Privacy Act) to request access to, deletion of, or correction of personal information we hold about you, and to opt out of the sale of personal information or its use for targeted advertising. We do not sell personal information. To exercise any privacy rights or ask questions, contact us using the information in “Contact Us” below.
9. State Consumer Health Data Laws
Some states, including Washington (My Health My Data Act), Nevada, and Connecticut, have enacted laws that specifically regulate “consumer health data” – information that identifies a consumer’s past, present, or future physical or mental health status, separate from HIPAA. Answers submitted through our Hormone Quiz may qualify as consumer health data under these laws if you are a resident of, or we are deemed to do business in, one of those states. Where these laws apply, we will not collect consumer health data without your consent, will not share or sell it for advertising purposes without your authorization, and will honor requests to withdraw consent or delete consumer health data you have provided. 10. Data Retention
We retain contact-form and quiz information for as long as needed to respond to your inquiry, follow up regarding our services, and comply with legal and record-keeping obligations. Information that becomes part of your medical record, once you are a patient, is retained according to the timelines described in our HIPAA Notice of Privacy Practices and applicable law.
11. Children’s Privacy
This Site is not intended for independent use by anyone under 18. We do not knowingly collect personal information directly from a visitor under 18 who is using the Site on their own (for example, a minor completing our Hormone Quiz or Contact Us form without a parent or guardian). However, we do treat patients under 18 when a parent or legal guardian submits information and requests services on the minor’s behalf. If you are a parent or legal guardian doing so, you represent that you have the legal authority to consent to health care services for that minor. Information submitted on behalf of a minor patient is handled as part of that patient’s care in accordance with our HIPAA Notice of Privacy Practices and applicable state law governing a minor’s health information and a parent’s or guardian’s access to it.
12. Data Security
We use reasonable administrative, technical, and physical safeguards designed to protect information submitted through the Site. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
13. Third-Party Links
The Site links to third-party sites we do not control, including our Patient Portal, social media pages, and affiliate product links. This Privacy Policy does not apply to those third-party sites, and we encourage you to review their privacy policies.
14. Changes to This Policy
We may update this Privacy Policy from time to time. The “Effective Date” above reflects the most recent revision. Material changes will be posted on this page.
