Holiday Hours and Last Day to Ensure Refill

The Revitalize Medical office will be closed from December 24, 2021 – to December 30, 2021, for Christmas. We will be back in the office on January 3, 2022. To ensure your refill request is completed, please have all refill requests submitted by December 22, 2021. Requests made beyond this date are not guaranteed to be fulfilled before January 3.

How To Request Refills

Contact your pharmacy and request an electronic refill request, and Revitalize will authorize this before the holiday break.

How To Access Lab Results From Home

If you have a lab review appointment over the phone, it is helpful to be able to see your lab results prior to or during this appointment. The easiest way to access your labs is through the Labcorp Patient Portal. If you have not already done so, please create a login (see steps below) We are no longer able to email each patient their individual lab results.
