Also, we are pleased to announce that Revitalize has grown and we have opened a second office space! The new space is just three doors down (west) from our current office. The address is 11576 South State Street, Suite 103A in Draper, UT 84020. Patients of Nikki Kelly, Michelle Diehl, and Kecia Chandler will be seen in this new office space. Lab draws will continue to be in the original office.

Below are the 2022 Holiday Season closures for Revitalize:

Thanksgiving: Wednesday, November 23, 2022 – Friday, November 25, 2022

Christmas: Friday, December 23, 2022 – Monday, January 2, 2023.

If you are running low on prescriptions, please submit your refill requests to your pharmacy at least 72 hours before the holidays!

Dexa Body is coming to Revitalize!

Dexa Body will bring a mobile scan unit to Revitalize every month!
